New Client Registration
Dr. Shirin participates in several insurance plans through major carriers. To become a counseling client, please click the link below and complete the New Client Registration Packet in full. Once the packet is submitted, our benefits coordinator will verify your insurance. After verification, Dr. Shirin will email you to set up an appointment.

How to Schedule
Complete the New Client Registration Packet (link below), including the insurance form in its entirety so we can verify your benefits before your first session. We do not receive the packet until all forms are submitted, so please complete the form in its entirety to schedule an appointment.
After benefits are verified, we’ll contact you to schedule.
